Psoriasis Registry

Thank you for your interest in our Psoriasis Registry.  Please read the following information about the study to see if you qualify.
  
Inclusion Criteria: (Must be able to answer “YES” to the following)

• Willing to commit for 6 month follow-up visits for 8 years (two visits per year)
• Have a diagnosis of Psoriasis
• Are a candidate for or are currently receiving conventional systemic agents (examples: Methotrexate, Acitretin, or Cyclosporine) or receiving biologic therapy for psoriasis (examples: Remicade, Enbrel, or Humira).
• Age 18 years or above

Exclusion Criteria:  (Must be able to answer “NO” to the following)

• Are participating or planning to participate in a clinical trial with an investigational agent.

**Please note that this is clinical trial registry. There will be no medication dispensed for this study.  You must already be taking one of the medications mentioned above in the Inclusion criteria.  You will come to our office every 6 months and your psoriasis and quality of life will be assessed by our Dermatologist and reported to the Sponsor of the study. Your information will always be kept confidential. There will be compensation for your time and travel of $45.00 per visit**
